Community Amenities and Services

Caregivers are available on-site 24 hours a day to provide a variety of services, such as medication management, diabetic care, and assistance with incontinence. For those who need help with their daily routines, we also offer assistance with activities of daily living, like dressing, grooming, and bathing. Our care plans are created based on your own unique medical, physical, mental, and spiritual needs. We recognize that no two people are exactly the same, so why should they have the same care plan?

Living Options

For those in assisted living, there are three different floor plans to choose from, ranging from studio apartments, to one- and two-bedroom suites. All units include a spacious eat-in kitchen area, generous storage space, and in-unit washers and dryers.

Residents in our memory care community can choose from three different studio floor plans. Many units include a cute kitchenette area for your convenience, as well as large windows which allow an abundance of natural light.

All apartments include bathrooms equipped with walk-in showers, grab bars, motion-sensored lights, and an RCare Safety System. Enjoy the privacy of your own space while knowing that assistance is available if an emergency should arise.

Care Types Offered

Azura Assisted Living and Memory Care of Brookfield is a community based residential facility (CBRF), which in the state of Wisconsin is a facility that provides care, treatment, and other services to five or more unrelated adults who need supportive or protective services or supervision. CBRFs are limited to those who do not require care above intermediate nursing care or more than three hours of nursing care per week. CBRFs are categorized by size, the resident's ability to evacuate, and disability/condition or status.
