Experience a new type of memory care at Azura Memory Care of Oak Creek – a cozy community located just a few miles from Lake Michigan in Oak Creek, Wisconsin. Our Oak Creek campus includes two communities in close proximity that are purposely designed for those living with cognitive impairments. With our MOSAIC amenities, like a sound-masking system and a multi-sensory room, residents will experience a type of living and care that they won’t find anywhere else. We also encourage residents to get outdoors while remaining safe in our enclosed courtyard, which features walking paths, stunning gardens, and nice seating areas.
At Azura Memory Care of Oak Creek, our caregivers are trained in accordance with the Alzheimer’s Association’s Guidelines for Dignity as well as our own exclusive MOSAIC training, which teaches our team to effectively engage and communicate with individuals living with dementia. This combination of training gives our team the unique expertise that can only be found at Azura Memory Care.
Community Amenities and Services
The aging process affects everyone at different rates and in a variety of ways, especially for those with memory loss. That is why at Azura Memory Care of Oak Creek, we get to know each individual’s needs, preferences, and histories, so that we may create a customized care plan that is specifically tailored to fit their unique needs. Caregivers are available around the clock, 24 hours a day, to provide assistance with daily living activities, medication management services, and diabetic and incontinence care. We understand that some days are better than others, which is why behavioral and medical needs are monitored daily to help ensure each resident receives the care they need. Our community collaborates with local physicians, podiatrists, therapists, psychiatrists, and other specialists to provide any number of services a resident may need in order to live their best life.
With home-cooked meals prepared daily and regular housekeeping and laundry services, residents of Azura Memory Care no longer have to stress over daily chores, and instead, can focus on stimulating their brain through a variety of activities and therapies offered on-site. One of our many MOSAIC therapy programs is active therapy. Incorporating daily exercise classes, such as noodle ball or yoga, helps invigorate both the body and the mind. Outdoor activities are planned on a regular basis, allowing residents to reconnect with past memories and hobbies.
Other activities, such as painting, cooking classes, gardening club, and trivia nights are organized throughout the week to keep everyone active and engaged. We also coordinate religious services and activities, including a Bible study, for those that want to continue their spiritual life. Living Options
At the end of a busy day, every resident can relax in their own private studio suite. Enjoy the convenience of a private bathroom equipped with a walk-in shower, an emergency response system, grab bars, and other safety features. Each suite also has built-in cabinetry and cable TV hook-ups. For couples, or even for those who would just like more space, we offer enhanced suite options as well. Feel free to decorate with family photos and other keepsakes to make it feel just like home.
Care Types Offered
Azura Memory Care of Oak Creek is a community based residential facility (CBRF), which in the state of Wisconsin is a facility that provides care, treatment, and other services to five or more unrelated adults who need supportive or protective services or supervision. CBRFs are limited to those who do not require care above intermediate nursing care or more than three hours of nursing care per week. CBRFs are categorized by size, the resident's ability to evacuate, and disability/condition or status.
