Situated between Lake Butte des Morts and Lake Winnebago is Azura Memory Care of Oshkosh, where we offer exceptional, personally-tailored memory care services for those living in the Oshkosh, Wisconsin area. At Azura Memory Care of Oshkosh, our memory care teams are specially trained to care for those with cognitive impairments, in accordance with the Alzheimer’s Association's Guidelines for Dignity. Our caregivers also undergo our unique MOSAIC education program, where we train our team how to properly communicate with residents as well as how to manage behavioral expressions.

Community Amenities and Services

By keeping our caregiver-to-resident ratio high, our team is able to get to know each individual on a one-on-one basis. Not only does this allow us to provide services that fit their personal needs, like diabetic or incontinence care, but we also learn their preferences and histories. Our caregivers then use this knowledge to create customized activities to increase engagement and quality of life. This unique understanding also helps our teams provide other services, like assistance with daily living activities and medication management.

We understand that residents may require some services, like physical therapy or end-of-life care, that we do not offer at our community. That is why we coordinate with local specialists, therapists, and hospice providers to ensure individuals receive the necessary care they need. We also bring in visiting psychiatrists, physicians, optometrists, and other specialists regularly, so residents don’t have to worry about organizing transportation to appointments.

Through our MOSAIC programming, residents have access to a variety of amazing therapies and activities, such as music therapy. Music can resonate feelings of joy and even provide moments of recollection, which is why we intertwine music therapy into our daily activities. We do this by bringing in live entertainment, scheduling karaoke nights, or simply by allowing residents with musical talents to play instruments. Other engaging activities, like art classes, outdoor activities, and off-site trips, are also organized, to keep everyone physically, mentally, and socially active.

Care Types Offered

Azura Memory Care of Oshkosh is a community based residential facility (CBRF), which in the state of Wisconsin is a facility that provides care, treatment, and other services to five or more unrelated adults who need supportive or protective services or supervision. CBRFs are limited to those who do not require care above intermediate nursing care or more than three hours of nursing care per week. CBRFs are categorized by size, the resident's ability to evacuate, and disability/condition or status.
