Our memory care team is available around the clock, 24 hours a day, to provide services such as medication management, diabetic and incontinence care, and assistance with daily living activities. For those with higher medical needs, like physical or occupational therapy or end-of-life care, we collaborate with local physicians, therapists, and hospice providers in order to help ensure that each resident receives services that best benefit their current and future needs. Podiatrists, psychiatrists, and other specialists visit the community regularly to take care of a variety of physical and mental health needs.

Many unique therapies and activities are available at Azura Memory Care of Stoughton, due to our MOSAIC program. For example, our Cognitive Therapy encourages residents to exercise their brains in a variety of ways, such as with BINGO, chess, and card games. Other recreational and social activities, like crafts and exercise classes, are available daily to keep everyone active and engaged.

We currently offer cozy studio and one-bedroom apartments for residents to choose from. With regular housekeeping and laundry services, each living space will stay neat and tidy, making it the perfect space to relax at the end of the day. Our bathrooms include many safety features for your convenience. We also provide RCare pendants for residents to use in case an emergency should ever arise.

Care Types Offered

Azura Memory Care of Stoughton is a community based residential facility (CBRF), which in the state of Wisconsin is a facility that provides care, treatment, and other services to five or more unrelated adults who need supportive or protective services or supervision. CBRFs are limited to those who do not require care above intermediate nursing care or more than three hours of nursing care per week. CBRFs are categorized by size, the resident's ability to evacuate, and disability/condition or status.
