Assisted Living Services

Hart Park Square provides supportive care to maximize an individual's health and wellness, such as:

• Medication management and administration
• Health and symptom monitoring as ordered by the physician for conditions such as diabetes, congestive heart failure and Parkinson's Disease
• Routine monitoring of weight change and blood pressure
• Assistance with bathing, dressing, showering, grooming, respiratory equipment and bathroom assistance to the degree necessary to support independence
• Apartment checks, reminders and escorts provided to encourage participation in leisure time programming and meals as indicated by the individual service

Staff is on duty 24 hours per day. A full-time registered nurse is on staff and provides clinical oversight. Support with chronic conditions, and on-site medical services, (x-ray, lab services, vaccinations, therapies, pharmacy and hospice) are available. Hart Park Square is wheelchair accessible.
