Heritage Lane is a family-run community, and we think of our residents as an extension of the family - treating everyone with respect, love, and dignity. We believe in coordinated team care for you or your loved one and believe family involvement is paramount. The owners and staff work together in creating a unique, personalized, and structured care plan for each of our residents, and looking after the most vulnerable of our community members.
Our Community
Unlike a lot of other assisted living communities, we are located right in the thick of it all, within a block of shopping, leisure, and recreational options. We take advantage of our location by offering frequent day trips and excursions as safety allows. Our facility was renovated in 2008 to update for our residents’ comfort. We have an on-site beauty salon, activities room, and gathering areas.
Our seniors enjoy prepared meals as part of their daily routine - we employ local culinary workers to make healthy food options that benefit an active lifestyle.
Medical Care
We understand the needs of our clients who require Assisted Living. Our staff are trained to provide round-the-clock care should the need arise. Heritage Lane offers Assisted Living Services, it’s important to make sure those seeking care understand what that term defines. In the state of Arizona, the phrase Assisted Living Facilities represents multiple levels of care and licensed facilities, such as the following:
- A residential care institution provides supervisory care services, personal care services or directed care services on a continuing basis.
- Directed care services are programs and services, including personal care services, provided to persons who are incapable of recognizing danger, summoning assistance, expressing need or making basic care decisions.
- Personal care services include assistance with activities of daily living that can be performed by persons w/o professional skills or professional training and includes coordination or provision of intermittent nursing services and the administration of medications and treatments by a licensed nurse.
- Supervisory care services include general supervision, including daily awareness of resident functioning and continuing needs, the ability to intervene in a crisis and assistance in the self-administration of prescribed medications.
- An adult foster care facility can house 1-4 residents. An Assisted Living Center can house 11 or more residents while an Assisted Living Home can house 10 or less residents.
We have third-party medical services available as well - we have a dentist come in for scheduled check-ups and emergencies, a visiting podiatrist, an on-call doctor, a visiting nurse, and a home care agency onsite. You can rest assured that we are able to provide adequate care for our seniors.
Care services include a program to test insulin levels, 1-person assisted transfers, and incontinence care for bladder- and bowel-related symptoms. We are capable of providing care to residents with behavioral issues and our staff is specially trained in de-escalating behavioral episodes.
Living Arrangements
We offer 1-bedroom units that have wheelchair accessible units as well as companion units to those residents who would like to live with a roommate. Our community is air conditioned and temperatures are regulated to optimize comfort and safety. Some resident rooms feature roll-in and wheelchair accessible showers.
Religious Services
We offer devotional services on-site, so residents can practice or worship in the privacy of our community. Catholic and Protestant religious services are available, provided by a visiting chaplain. There are also several churches located within the surrounding neighborhood.
Move In Assistance
We understand that there may come a point in which seniors can no longer stay in their homes. In these cases, we start by sending out our team to evaluate our prospective residents. The team consists of Heritage Lane’s Director of Psychiatric Services, Director of Clinical Services, and Business Office Manager. Our Business Director is present to make sure your finances are arranged and sorted to relieve any stress you may have. Once you’re approved we have a physician recommend all of the necessary medical equipment and help set up for our incoming senior to be as comfortable and healthy as possible. On your move-in day, we have a peer volunteer meet with you and your family to ease your transition into your new community. You’ll be visited again by the Evaluation Team as part of your orientation, you’ll also meet with our Dietary Services Manager, Activities Director, and Director of Nursing for further specialized evaluation.
