Ann Cook Center

Ann Cook Center is a state-licensed Adult Foster Care located in the city of Saint Clair Shores, MI, just minutes away from Lake Saint Clair. We are located on a quiet residential street, but close to the city strip with easy access to shopping, restaurants, parks, hospitals, and churches whenever we feel the need to get out and explore.

Our AFC caters to individuals diagnosed with End-Stage Renal Disease; offering on-site Home Hemo and Peritoneal Dialysis for the convenience of our residents. Our home dialysis program uses the gentlest forms of therapy to preserve vital organs and reduce recovery time. We pair each resident with a companion that comes to the home and stays with them for the duration of each treatment at no extra out-of-pocket cost to them.

Our AFC and dialysis company is owned operated by a Registered Nurse Administrator. Tk is on-site every day and personally oversees every aspect of care-- from attending all doctor's appointments right down to the foods our residents eat and the amount of fluids they drink. The support staff includes a Medical Director, Registered Nurse, Registered Dietician, Licensed Clinical Social Worker, and trained direct care staff. Our Licensed Clinical Social Worker is available to offer therapy to residents diagnosed with mental health disorders.

Our home has an open floor plan with lots of light throughout and great amenities to make our residents feel safe and welcomed.
