Through our exclusive Brookdale HealthPlus program, residents receive comprehensive care that’s right for them. We build relationships with doctors and nurse practitioners who can provide care on-site. Our specialized program allows you to receive support for chronic conditions such as diabetes, high blood pressure, congestive heart failure, and more. In addition, we manage urgent needs and provide annual wellness visits, immunizations, and health screenings. Residents also receive direct assistance with meals and diet, housekeeping, transportation, and home maintenance.
