At Creekstone, we offer a unique, mindful, and compassionate approach to memory care that inspires Elders to live a happy and dignified life: We believe it’s our obligation to accommodate the health and personal care needs of each Elders while also nurturing their human spirit. Our small, cozy household setting creates an intimate senior living environment dedicated to promoting independence and dignity.

Our Creekstone community is located in the charming village of Fairport, NY, which has consistently been ranked one of the best places to live in the country. You and your loved ones will enjoy the quaint New England village, which is filled with local shops, restaurants, and live entertainment. Creekstone is also only a 15-minute drive from downtown Rochester.

Service and Amenities

At Creekstone, we believe in a different kind of care: Memory care does not have to be cold and institutional. Our family-centered approach to memory care emphasizes the individual, and we tailor the senior living experience to each of our residents. We have no long corridors, no imposing medical equipment, and no bulky medication carts strewn about our community. Instead, our ranch-style family home is warm and inviting. We have a large, central living and dining room, an accessible kitchen, and a secure courtyard. Our residents have complete freedom to enjoy the outdoors in a safe and secure environment.

We refer to our residents as Elders, and they, along with the staff, become a family where all help to take care of each other and the home. Rather than have a traditional hierarchy found in most long-term care facilities, we offer a decentralized approach that shifts authority and resources towards those working closest with Elders. Central to our support system are the Shahbazim, who are the key staff members at Creekstone who assist Elders in managing personal care needs, building relationships, and maintaining the home. Other staff members include the home’s administrator and two Family Care Guides (nurse/care-managers) who provide clinical support.

We believe that the need for meaning and fulfillment is perhaps the greatest need among our Elders. That’s why we think it is our obligation not only to care for the health and well-being of our residents, but also to make their lives free of loneliness, helplessness, and boredom. Our team includes dedicated Enrichment Guides who plan a daily calendar of activities that focus on nurturing physical, social, and spiritual well-being.

Living Arrangements and Unit Features

The Memory Care Residences at Creekstone provide care for persons living with dementia. All Elders have a private room and bathroom. Enhanced memory care is provided in one of the households and includes additional staff support for Elders living with advanced dementia. Respite or short-term care is also provided based on availability.

    We had a good tour with Creekstone. It is a nice facility and was our second choice.

  • Just like living at home

    We chose Creekstone because we liked the model. It is not a medical model, it is a home. It is 4 houses that back up to each other to form a square. Inside the square is a patio that all the houses share and it has a fountain and gazebo, it is beautiful. The house has a beautiful kitchen with an open floor plan. Their staff to resident ratio is 1 to 5. Each resident has their own bathroom and they can decorate their rooms as they would like. The breakfast meals are not on a set schedule which is nice. After talking with the chef, he was very good about putting things on the menu that my mother likes, he is very accommodating. They have a lovely screened in front porch that can fit 14 people. They allow pets as long as the pet is one they have had for years, so my mom was able to bring her dog. They do also have to be able to mostly care for their pet. I would definitely recommend them to others who are looking.
