Our Alzheimer's and dementia care community, also known as Clare Bridge, is rooted in €œa person-centered approach€. That means we help our residents find a sense of belonging and purpose while also helping them preserve who they are. We also have our Clare Bridge Crossings program which is designed for individuals in the early stages of Alzheimer's or dementia. While not appropriate for all persons with dementia, this program is often a stepping stone between assisted living and our Clare Bridge program.

Did you know we offer both rental and Life Care options?

In the rental program, you pay a monthly rental fee based on the level of care and services you need€” with no buy-in fee. That means you or a loved one move into whatever level of care you need€”for instance, assisted living or skilled nursing.

For the Life Care program, you've got two costs to consider, and even more financial gains.  Let's start with the upfront buy-in fee. The amount of the fee depends on your floor plan, services, and the Life Care contract you choose. It's the buy-in fee that makes the Life Care guarantee possible.* The other cost is the monthly service fee, which is based on the type of accommodations and your level of care.
With our Life Care option, you can make your assets last longer with substantial health, financial and lifestyle benefits. Most important in that list is the care for life guarantee*. This unique benefit guarantees that even if your financial resources are eventually exhausted due to no fault of your own You'll still have the care you need.*

Whether you choose a rental or Life Care option, our community has the lifestyle you want and the care you need.

*The Life Care guarantee is subject to the terms and conditions of the Residency Contract.

  • Putting the Care in Continuing Care

For many reasons—dementia, resistance to moving, leaving her home of 50 years—we expected Mom’s move to The Gardens at Freedom Village to be challenging. The sales, administration, and nursing staff created a transition plan: a clear contract, quick turnaround of a room, hanging photos/curtains, and receiving Mom on her terms. While never easy, Mom’s reaction and her adjustment were much better than we had hoped—all because of the care demonstrated by the staff. Her room feels like "hers." We are incredibly happy to see Mom interacting with the staff and residents—not living alone. We selected The Gardens at Freedom Village after visiting 5 continuing care communities and working with Mom on moving for 2-1/2 years. The Gardens at Freedom Village provided the best value because we have peace of mind. Low employee turnover and well-maintained facilities were important, but the support of each member of the staff is what gives us comfort that Mom is receiving quality care.

  • The Staff at Freedom Village Helps Make The Transition Time As Painless As Possible

I would let others know that the staff are responsive and friendly to the families and residents. The community is very clean. They seem to have a lot of activities available for the residents. The dining attendants are very attentive to the residents. They are willing to work with the residents on requests for food.

  • Highly recommend

I just thought that the hospice nurse here was outstanding. Also, the person who ran the special care unit, [name removed] was excellent. I would absolutely recommend to others looking if your situation comes down to it.
